Who is responsible for your data
Teknikki operates this demonstration and is the data controller for any personal information you submit through it. Synergy Dental Clinic does not receive it.

What we collect and why
If you fill in the enquiry form we receive the name, phone number, and optionally the email address, preferred time, clinic and message that you type into it. We collect this only so the form can be demonstrated end to end.
The form is not connected to any dental practice, so nobody will contact you about dental treatment as a result of using it. Please do not enter real medical details or anything you would not want stored.
Our web server keeps standard access logs, including IP addresses, which are used to rate-limit the form against spam and to keep the site running.
How long we keep it
Enquiries submitted through this demonstration are held only for as long as the demonstration is live, and are deleted when it is taken down. They are not used for marketing, are not sold, and are not shared with third parties.
Cookies
This site sets no cookies and runs no analytics or advertising trackers. Nothing here follows you to other websites.
Your rights
Under UK data protection law you can ask us for a copy of any personal data we hold about you, ask us to correct it, or ask us to delete it. Contact us and we will action it, normally within a few days.
If you are unhappy with how we have handled your information you can complain to the Information Commissioner's Office at ico.org.uk.
